What is your financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money order, VISA, or Mastercard, Amex, or Discover. Please note, co-pays are due prior to seeing the doctor.

What insurance do you accept?

Our practice bills all major insurance plans including: Medicare, Blue Cross, Cigna, Universal Healthcare, BlueCare, Tricare, Signature Network, and HealthNet. Please note that we are not contracted with all insurance companies. Please feel free to call if you have any questions, 310-416-9700.

How do I get refills on my prescription?

Prescription renewals should be called into your pharmacy at least 3 days before your supply is exhausted. Have your pharmacy fax a refill request to our office, 310-416-1144 (Fax).

What are your office hours?

Monday to Friday from 9:00am to 5:00pm.

What should I bring to my appointment?

Please bring your insurance card, referral if required by your insurer, operative reports, test results, images (x-ray films, MRI, CT) and reports.
