Often times, our patients want to know what our financial policy is, and what kind of insurance we accept. Makes sense to us that patients want to ask those questions. That’s why we dedicated this blog to answering those frequently asked questions for our patients. Pls take a minute to read and maybe then print out the questions and answers we provide. What is our financial policy?
Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money order, VISA, or Mastercard, Amex, or Discover. Please note, co-pays are due prior to seeing the doctor.
What insurance do we accept?
Our practice bills all major insurance plans including: Medicare, Blue Cross, Cigna, Universal Healthcare, BlueCare, Tricare, Signature Network, and HealthNet. Please note that we are not contracted with all insurance companies. Please feel free to call if you have any questions, 866-691-4844.
How do I get refills on my prescriptions?
Prescription renewals should be called into your pharmacy at least 3 days before your supply is exhausted. Have your pharmacy fax a refill request to our office, 310-416-1144 (Fax).
What are our office hours?
Monday to Friday from 9:00am to 5:00pm.
What should I bring to my appointment?
Please bring your insurance card, referral if required by your insurer, operative reports, test results, images (x-ray films, MRI, CT) and reports.
